To address this question, we conducted a multicenter retrospective study including 487 patients treated across eight French academic institutions between 2014 and 2023. Among them, 182 patients (37.4%) underwent robotic-assisted BCE (RA-BCE) and 305 (62.6%) underwent open BCE (O-BCE).
Oncological Outcomes
After a median follow-up of 30 months, intravesical recurrence occurred in 23.2% of patients, with a median time to recurrence of 7.5 months. Importantly, the 3-year intravesical recurrence-free survival was similar between RA-BCE and O-BCE (68.9% vs 59.7%, log-rank p=0.1). Robotic BCE was not independently associated with bladder recurrence on multivariable analysis (HR 0.77, p=0.2). Similarly, 3-year extravesical recurrence-free survival (69.2% vs 63.8%,log-rank p=0.2), cancer-specific survival, and overall survival were also comparable between the two approaches. These findings remained consistent in sensitivity analyses excluding patients with prior bladder cancer.


Perioperative Outcomes
Beyond oncologic safety, robotic BCE was associated with improved perioperative outcomes. Patients treated with RA-BCE experienced significantly fewer overall 90-day postoperative complications (10.1% vs 21.3%, p=0.02), fewer major complications (4.7% vs 7.1%, p=0.02), and shorter hospital stay (5 vs 7 days, p<0.01). Duration of bladder catheterization was similar between groups.

Taken together, these findings suggest that robotic intracorporeal bladder cuff excision can achieve oncologic outcomes comparable to the open approach while preserving the perioperative benefits of minimally invasive surgery. Although prospective validation remains needed, our results support the growing role of fully robotic RNU in the management of localized UTUC.
