| The COMPPARE prospective study of 2,500 men with localized prostate cancer found excellent early outcomes with both IMRT and proton therapy, with nearly identical 3-year freedom from disease progression and no significant differences in patient-reported quality of life or reported toxicity. Rectal spacers substantially reduced GI grade ≥2 toxicity in both groups, while dosimetrically, proton therapy delivered ~50% lower dose to normal tissues, but this did not translate into early clinical toxicity differences.

These mature data support ultra-hypofractionation as an evidence-based standard-of-care option (particularly for intermediate-risk disease), with contemporary techniques expected to further reduce toxicity and future work focusing on risk-adapted intensification and integration with systemic therapies. |
