| In this plenary session, William Thomas Lowrance summarized the 2021 AUA/ASTRO/SUO guidelines on advanced prostate cancer, with key take-home points highlighted. He reiterated that these guidelines were produced by a multidisciplinary panel with representation from the AUA, ASCO, ASTRO, and SUO. They also included a patient advocate. The aims of the guidelines are to assist in clinical decision-making and provide evidence-based guideline recommendations across the entire continuum of advanced prostate cancer.

| In this presentation, Chris Bagma, focused on anxiety and quality-of-life for men on active surveillance for prostate cancer. Once diagnosed with low-risk prostate cancer and put on active surveillance, men can carry a heavy burden. The focus of the presentation is on anxiety and quality of life. Anxiety is a feeling of unease, such as worry or fear, that can be mild or severe. It can range from normal stress to pathologic condition and also from incidental to continuous – to the point it can affect daily life. |

| State-of-the-art Lecture: Personalized Medicine in the Management of Prostate Cancer Across the Patient Care Continuum

| The American Urologic Association annual meeting included a State-of-the-Art Lecture by Brian Chapin who discussed personalized medicine in the management of prostate cancer across the patient care continuum. Dr. Chapin notes that the way we think about the personalized approach to prostate cancer therapy involves seeing a patient in the clinic and assessing them on a number of factors (clinical features, genetics, genomics, serum markers, receptors, induced responses, and selection pressures) and then developing a personalized approach to their treatment plan.

| he advanced prostate cancer session at the 2021 American Urologic Association (AUA) annual meeting included a presentation by Dr. Scott Tagawa who discussed long-term follow-up and prognostic factor analysis among men with mCRPC who received 177Lu-PSMA therapy. Dr. Tagawa reported the results of a combined analysis of men with mCRPC treated on sequential prospective clinical trials of PSMA-targeted 177Lu. |

| Relugolix vs Leuprolide Effects on Castration Resistance-Free Survival from the Phase 3 HERO Study in Men with Advanced Prostate Cancer |

| Fred Saad discussed the effect of relugolix versus leuprolide on castration resistance-free survival (CRFS) among men included in the phase 3 HERO study. Relugolix achieved rapid, sustained suppression of testosterone levels that was superior to that with leuprolide in men with advanced prostate cancer. These results suggest that low testosterone levels on ADT are not a driver of early castration-resistance. |

| TOAD: Warts and All? Subgroup Analyses Revealing Differential Treatment Effects in the Timing of Androgen Deprivation Trial

| Kristian Stensland discusses the TOAD trial presenting a subgroup analyses which demonstrated that the overall survival benefit demonstrated in the TOAD trial may be significantly impacted by whether patients received ADT prior to trial entry. There are multiple possible explanations for this difference in addition to post-hoc trial selection bias, including issues surrounding potentiation of androgen sensitivity, with significant implications for treatment of relapsed prostate cancer and future study planning.

| In this study by Benjamin Lowentritt, he evaluates the real-world effectiveness and treatment adherence of apalutamide in men with non-metastatic castration-resistant prostate cancer (cM0 CRPC / nmCRPC). Based on the SPARTAN trial, apalutamide was approved for use in conjunction with androgen deprivation therapy (ADT) for men with cM0 CRPC.
