| Sima Porten discussed adjuvant therapy after nephroureterectomy for high-risk upper tract urothelial carcinoma. Patients at high risk of recurrence after definitive surgical treatment for upper tract urothelial carcinoma should be referred for consideration of adjuvant therapy, including patients with (i) high grade pT2-pT4 and/or pN+ disease, and (ii) if neoadjuvant chemotherapy was given, ypT3-4a and/or ypN+ disease. There is level 1 evidence for adjuvant chemotherapy (gemcitabine + platinum). |

| Max Kates discussing rescue therapy and BCG alternatives in non-muscle invasive bladder cancer (NMIBC). Dr. Kates started his presentation by noting that the United States now has one supplier of BCG, with three major shortages in the past decade. The implications of these BCG shortages have been assessed in previous studies, reporting that there are significant differences in recurrence free survival among patients treated during the BCG shortage compared to patients treated during times of normal supply.

While radical cystectomy remains the preferred treatment for bacillus Calmette-Guérin (BCG) unresponsive high-risk NMIBC, many patients are either unwilling or unfit to undergo surgery. Previous retrospective studies have demonstrated the efficacy of intravesical gemcitabine and docetaxel for treating NMIBC after BCG failure but only reported moderate-length follow-up.
