| Imad Ziouziou presented on the preparation of patients for radical cystectomy. Dr. Ziouziou emphasized that we must check everything before taking the decision to proceed with radical cystectomy, including detecting frailty, anticipating surgical complications (inform the patient), anticipate general complications in collaboration with anesthetist and other colleagues, check that the environment (hospital) is appropriate, and implement ERAS. The decision ultimately comes down to a shared decision-making process with the patient, oncologists, anesthetists, cardiologists, and perhaps other surgical colleagues.

Dr. Kassouf emphasized that many of his recommendations will be based on the new CUA Guidelines on the management of non-muscle invasive bladder cancer 2021, which is currently in press at CUAJ. |

| The Canadian Urological Association joint session with the European Association Urology (EAU) at the 2021 annual EAU virtual meeting included a presentation by J. Alfred Witjes who discussed shorter maintenance regimens for patients receiving BCG therapy. When the guidelines discuss BCG “and/or alternative treatments”, this is an issue for patients with higher-end intermediate-risk NMIBC, high-risk NMIBC, the highest risk patients, and those that are failing BCG. |

| Treatment Challenges to BCG Unresponsive Non-Muscle Invasive Bladder Cancer |

| Keisuke Shigeta from Tokyo, Japan joined the Joint Session of the European Association of Urology and Japanese Urological Association at the 2021 EAU annual meeting to discuss treatment challenges in BCG unresponsive non-muscle invasive bladder cancer (NMIBC). Radical cystectomy is the only promising treatment option for cure in BCG unresponsive NMIBC patients, however clinical trials including intravesical PD-1/PD-L1 blockade are ongoing to provide bladder preserving options for these patients. |
