| At ASCO 2021, Mark Fleming discussed treatment sequencing in advanced prostate cancer within a session entitled “Beyond the Androgen Receptor: New Avengers in the Treatment of Castrate-Resistant Metastatic Cancer”. Dr. Fleming highlighted the importance of previous treatment administered, metastatic site, and symptoms in guiding treatment choice. However, there is no definitive sequence and treatment needs to be personalized to patient, disease, and treatment considerations. As we move forward, treatment will be increasingly guided by a biologic understanding of disease.

| Rahul Aggarwal presented data assessing whether a single dose of Lu can induce an immunogenic priming effect to improve outcomes of men with mCRPC subsequently treated with the immune checkpoint inhibitor pembrolizumab. A single priming dose 177Lu-PSMA-617 followed by pembrolizumab was well tolerated and leads to durable responses in a subset of mCRPC without high mutational burden or microsatellite instability. |

| Kim van der Zande presented results of the OSTRICh trial which randomized patients with poor-prognosis mCRPC following progression on docetaxel to cabazitaxel or an androgen receptor-targeted agent. There was no significant difference between the clinical benefit of cabazitaxel and androgen receptor-targeted agents in this population of patients with poor-prognosis mCRPC though radiographic assessment favored patients treated with cabazitaxel and PSA-based metrics favored patients treated with androgen receptor-targeted agents. |

| Emeline Colomba presented results of the ODENZA trial to assess whether differences in toxicity affect patient preference among those with early mCRPC. A numerically greater number of patients with early mCRPC preferred darolutamide compared with enzalutamide, though this was not significantly different. Fatigue was a key factor influencing patient preference. |

| Fernando López-Campos evaluated the significance of an early PSA increase in mCRPC patients treated with enzalutamide or abiraterone. PSA progression at 4 weeks after enzalutamide or abiraterone is significantly associated with shorter OS and may help identify patients not benefitting from enzalutamide or abiraterone before clinical or radiographic progression. PSA pattern progression and its association with OS might differ depending on the drug used (enzalutamide or abiraterone). |

| Niven Mehra presented results of the TALAPRO-1 safety profile of talazoparib in men with mCRPC with the aim of understanding how patients with adverse events were managed during the trial. TALAPRO-1 is a single-arm, open-label, phase 2 study of talazoparib in patients with progressive mCRPC, measurable soft tissue disease, and DNA damage response mutations likely to sensitize to PARP inhibitors, who received ≥1 taxane-based chemotherapy and progressed on ≥1 novel hormonal therapy. |
