| The Phase III PROpel Trial in mCRPC, A Biomarker Analysis and the Detection of Mutations in HRR Genes
|
Noel Clarke, MBBS, FRCS, ChM
Noel Clarke joins Alicia Morgans in discussing an updated analysis and updated results from the Phase III PROpel trial of abiraterone and olaparib vs abiraterone and placebo as first-line therapy for patients with metastatic castration-resistant prostate cancer (mCRPC).

| BRCAAway - A Randomized Phase 2 Trial of Abiraterone, Olaparib, or Abiraterone + Olaparib in Patients with mCRPC with DNA Repair Defects
|
| Maha Hussain, MD, FACP, FASCO
|
| Maha Hussain discusses BRCAAway with Alicia Morgans, based on Dr. Hussain's presentation at ASCO 2022. Dr. Hussain concluded that the randomized phase 2 BRCAAway, by emphasizing that in mCRPC patients with inactivating BRCA1, BRCA2, and/or ATM alterations, abiraterone + prednisone + olaparib was well tolerated and resulted in longer PFS and better PSA response versus either agent alone.

| Are There Any Other PARP Inhibitor Combinations in Prostate Cancer We Should Be Exploring?
|
| Evan Yu, MD
|
| Evan Yu discusses ongoing trials that combine a PARP inhibitor with novel approaches. We now have a role for single agent PARP inhibition for biomarker-selected patients with mCRPC and a potential future role in combination with abiraterone acetate for first-line mCRPC treatment. However, we should remain aggressive in testing the provocative novel combinations below.

| Gene-by-Gene Analysis in the MAGNITUDE Study of Niraparib with Abiraterone Acetate and Prednisone in Patients with mCRPC and HRR Gene Alterations |
| Shahneen Sandhu, MD, Ph.D., MBBS, FRACP |
| Shahneen Sandhu discusses a gene-by-gene analysis in the MAGNITUDE study of niraparib + abiraterone acetate + prednisone in patients with mCRPC and HRR gene alterations. These data support the overall conclusions of the MAGNITUDE primary analysis and support the benefit of niraparib + abiraterone acetate + prednisone in patients with HRR gene alterations, both BRCA1/2 and beyond BRCA1/2. |

| Discussant: AR and PARP: Partners in Crime |
| Heather Cheng, MD |
| Heather Cheng discusses three important abstracts incorporating inhibition of AR + PARP in advanced prostate cancer. The first, “BRCAAway: A randomized phase 2 trial of abiraterone, olaparib, or abiraterone + olaparib in patients with mCRPC with DNA repair defects” presented by Dr. Maha Hussain, “Tolerability of abiraterone combined with olaparib in patients with mCRPC: Further results from the phase III PROpel trial” presented by Dr. Antoine Thiery-Vuillemin, and “Gene-by-gene analysis in the MAGNITUDE study of niraparib with abiraterone acetate and prednisone in patients with mCRPC and HRR gene alterations” presented by Shahneen Sandhu. |

| Maha Hussain discusses the BRCAAway trial, a randomized phase 2 trial of abiraterone, olaparib, or abiraterone + olaparib in patients with mCRPC with DNA repair defects. Dr. Hussain concludes her presentation discussing the randomized phase 2 BRCAAway by emphasizing that in mCRPC patients with inactivating BRCA1, BRCA2, and/or ATM alterations, abiraterone + prednisone + olaparib was well tolerated and resulted in longer PFS and better PSA response versus either agent alone.

| Tolerability of Abiraterone Combined with Olaparib in Patients with mCRPC: Further Results from the Phase III PROpel Trial |
| Antoine Thiery-Vuillemin, MD |
| Antoine Thiery-Vuillemin discusses further results from the PROpel trial assessing tolerability of abiraterone combined with olaparib in patients with mCRPC. The overall results of PROpel demonstrate the clinical benefits of olaparib in combination with abiraterone in the first-line treatment of an unselected population of patients with mCRPC. |

| Olaparib Plus Abiraterone As First-Line Therapy in Men With Metastatic Castration-Resistant Prostate Cancer: Pharmacokinetics Data From the PROpel Trial
|
| Andrew Armstrong MD, MS
|
| At the 2022 American Society of Clinical Oncology Annual Meeting held in Chicago and virtually, the poster session focused on Prostate, Testicular, and Penile Cancers included a presentation from Andrew J. Armstrong discussing the pharmacokinetics of combined olaparib and abiraterone treatment in men with previously untreated metastatic castration-resistant prostate cancer (mCRPC) treated in the PROpel trial.
