| Brian Rini gave a presentation focusing on the management of metastatic non-clear cell renal cell carcinoma (nccRCC). First, he focused on papillary renal cell carcinoma (RCC). Non clear-cell RCC is a mixed bag of histologies, biologies, and responses to therapy. VEGF and mTOR targeted therapy have minimal (but not 0!) activity as monotherapy.

| Ithaar Derweesh presents the role of cytoreductive partial nephrectomy in the setting of metastatic renal cell carcinoma. He does this through the context of a case presentation. In an era characterized by more effective systemic therapies with better oncologic outcomes (and longer survival), CPN for imperative indications is safe and feasible for larger and complex renal masses. Long-term impact of functional decline in metastatic patients is not characterized.

He starts by highlighting a collaborative review he and I worked on for European Urology that walks through decision making for the management of localized solid renal masses – where a key decision point is the choice to treat the tumor, and at which point, we have to choose between radical nephrectomy, partial nephrectomy, and ablative options. |

In this presentation, Srikala Sridhar gave a talk on the role of neoadjuvant immunotherapy for muscle-invasive bladder cancer. IO therapies have significantly changed the landscape. They are already approved in the NMIBC, adjuvant, and first/late-stage metastatic urothelial carcinoma settings. The only setting they have not yet been approved for is in the neoadjuvant setting.

| Katie Murray presents the optimization of care of bladder cancer in women. The topic is broader than just the optimization of care. She notes that there are significant gender disparities that need to be addressed, but she also focuses on organ-sparing cystectomy, vaginal wall-sparing cystectomy, and sexual function after cystectomy. |

| Debate: Renaming Gleason 6 Prostate Cancer

| In this presentation regarding the renaming of Gleason 6 Prostate Cancer, William Catalona started the debate with the CON side of the, while Dr. Matthew Cooperberg followed with the PRO. GG1 is an exceptionally common feature of aging, and in 2023 nearly universally represents overdiagnosis, effectively an incidental adverse effect of screening intending to find GG >= 2 cancers. It is argued that this change would dramatically improve the benefit to harm ratio for screening, and likely improve primary care uptake of screening, with mortality benefits far more than outweighing missed progression events.
