| Clara Hwang presented an analysis of biomarker-directed therapy in men with metastatic castration-resistant prostate cancer (mCRPC). In a real-world mCRPC molecular profiling cohort, there are similar overall rates of actionable molecular alterations in non-Hispanic black and non-Hispanic white men, but higher rates of MMRd/MSI-H and lower frequency of PTEN alterations in non-Hispanic black men. Further, there was not evidence of differences in the delivery of targeted therapy.

| Racial Concordance and Trust in Health Communications: A Randomized Trial of Videos About Prostate Cancer

| Stacy Loeb presented a randomized trial of 2904 U.S. adults age ≥40 that sought to evaluate the association between racial representation in online content about prostate cancer and trust in the content. A secondary objective was to identify additional attributes that influence trust in online content. While there is no difference among White adults, there is a significant association between racial concordance of the presenter and trust in prostate cancer information among Black adults.

Tamim Niazi described the rationale, design, and protocol of the DASL-HiCaP trial of darolutamide, along with androgen deprivation therapy, for patients undergoing definitive or salvage radiotherapy. The primary endpoint of this trial is metastasis-free survival (ICECaP-validated), with numerous secondary endpoints including overall survival, prostate cancer-specific survival, PSA-progression-free survival, time to subsequent hormonal therapy, time to castration-resistance, frequency and severity of adverse events, health-related quality of life, fear of recurrence.

The primary endpoint of this phase I trial is safety (incidence and severity [grading per NCI-CTCAE V5.0] of AEs including DLTs). Secondary endpoints include prostate-specific antigen response rate, overall response rate (PCWG3 modified RECIST 1.1 criteria), PK, PD, immunogenicity, and biomarker analyses.

In high-risk patients, biochemical recurrence after salvage radiotherapy reaches nearly 50%. Despite improved post-salvage radiotherapy control from RTOG consensus fields and risk-management recommendations, relapse rates remain as high as 50% at 5 years for high-risk patients.

In this presentation, Alicia Morgans described the progression patterns of patients with non-metastatic castration resistant prostate cancer (nmCRPC) in the ARAMIS trial. While darolutamide significantly reduced risk of metastatic progression and improved overall survival in men with nmCRPC, it did not change the pattern of metastatic progression. Further, importantly, many patients with nmCRPC experienced metastatic progression without PSA progression, and pain progression was rare.
