Nima Nassiri, MD et al. review a 90 year old patient taken for intraoperative blue light cystoscopy, which revealed multiple lesions that were then resected. Pathology revealed high grade urothelial carcinoma with lamina propria invasion and the patient was managed successfully with BCG therapy. Their commentary highlights their treatment decisions, the role for BLC in the initial diagnosis, and unanswered questions regarding the role of BLC in patients with non-muscle invasive BC at high risk for progression to invasive disease.
