WCE 2017: Tumor Location and Its Association with Non-Organ Confined Upper Tract Urothelial Carcinoma

Vancouver, Canada (UroToday.com) Dr. Derek Gearman, resident of urology at Mayo Clinic, presented data on the topic of tumor location as an important risk factor for higher stage disease in patients with upper tract urothelial carcinoma (UTUC). The purpose of this study was to discern whether tumor presence in the ureter or renal pelvis is correlated with worse oncologic outcomes. 

For analysis, Dr. Gearman and his team retrospectively reviewed 343 patients from their UTUC database between 1995 and 2009. Utilizing the Kaplan Meier and Cox Proportional hazard logistic model for evaluation, 114 patients had a tumor located in the ureter, and 229 patients had a tumor located in the pelvis. 

The outcomes of this study reveal that 32.8% of patients with a renal pelvis tumor and 23.7% of patients with ureteral tumors had non-organ confined disease, respectively. 

Dr. Gearman concluded that patients with renal pelvis tumors are at a greater risk for higher rates of non-organ confined disease than patients with ureteral tumors. Based on these initial results, tumor location is indeed an important prognostic factor to consider when deciding the best course of treatment. Dr. Gearman added that for future direction, prospective studies are needed to further evaluate these conclusions.
