SUO 2016: Limiting overuse in asympomatic microhematuria evaluation, is there a safe way forward? - Session Highlights

San Antonio, Texas USA ( Matthew Nielson, University of North Carolina, discussed hematuria workup and guideline recommendations. In Europe, imaging may include ultrasound with increasing concerns regarding radiation exposure of computerized tomography urograms (CTU).

In a recent study out of Kaiser Health System, overall cancer diagnosis among microhematuria patients was 2.9%. AUA pooled data analysis was similar. There has been discussion regarding risk stratifying microhematuria workup with increasing use of cystoscopy and CTU based on increasing number of risk factors. Cost-effectiveness research has further supported revision of our current guidelines and supports risk stratification to microhematuria workup. Given lack of upper tract findings on microhematuria workup with increasing costs associated with CTU in the current AUA guideline recommendations, there is a need to limit overuse of this imaging modality. Furthermore, radiation exposure is not to be underestimated with inherent risks of secondary malignancy which should not be ignored.
