SUFU 2021: Underactive Bladder (UAB)/Detrusor Under-activity (DUA)

( Alan Wein provided some personal thoughts about underactive bladder (UAB) and detrusor under-activity (DUA). His view is that “we have a common condition that we think we can recognize but cannot exactly define or quantitate which may be a contributing factor in the failure to empty and sometimes in failure to store with little data on natural history and no agreed-upon indications for treatment except for urinary retention and no established agreed-upon routinely successful therapy”.

Ideally, UAB and DUA should be similar with a symptom or a set of symptoms that characterize UAB and tests for diagnosing DUA but they need to be specific for age, gender, and maybe etiology. He also feels these conditions should be characterized with a score or scale of severity and their effect on QoL, particularly for UAB. We do not really know the consequences or the natural progression of these two conditions. Dr. Wein questioned treating a patient with DUA/UAB symptoms of urinary flow (hesitancy, slow stream) or sensation if retention or residual urine is not present. So UAB is defined by non-specific symptoms and it may or may not be associated with DUA. But DUA is complicated as it differs based on age, sex, and etiology of the DUA.
