AUA 2017: Best Abstract – Are the current follow-up guidelines after treatment for organ confined renal cancer sufficient?

Boston, MA (UroToday.com) This study raises a provocative question – Are the current followup guidelines adequate after partial nephrectomy. The authors retrospectively queried 880 patients undergoing partial nephrectomy for RCC less than pT2b. In long term followup, recurrence was found in 4.4% of the patients, consistent with previous reports. However, 42% of all recurrences occurred after five years of followup. This alarming late-recurrence rate underscores the lack of guideline recommendation for long term follow up of RCC after partial nephrectomy beyond five years. Furthermore, the authors found a trend towards late recurrence in patients with low grade and low stage tumors. In addition, patients with lung metastases and multiple metastatic lesions tended to develop metastases earlier than those with isolated bone or adrenal metastases, raising the possibility of organ tropism for slow growing RCC metastases.
