WCE 2007 - Technique to Decrease Ischemia Time During Laparoscopic Partial Nephrectomy With Early Functional Results

Dr. Nguyen and colleagues from the Cleveland Clinic Foundation presented their technique and outcomes with respect to early unclamping of the renal hilum (i.e. prior to the placement of one or sometimes no parenchymal sutures. Remaining sutures are placed on an as needed basis following vascular clamp removal. Fifty patients using this technique were compared to 50 patients using the authors standard technique described elsewhere. The warm ischemia time in the early unclamping group was 13.1 minutes vs 31.1 minutes in the standard technique group with no significant difference in the overall operative complication rate. Importantly, there was no significant difference in the GFR when estimated using an abbreviated MDRD formula.

The group concluded that this technical modification did not result in increase blood loss or operative time, but significantly decreased the warm ischemia time.
