Prostate Biopsy and Optimization of Cancer Yield
Wednesday, 01 March 2006 It is hard to imagine practicing urology and urologic pathologic today without prostate-specific antigen (PSA) and multiple thin-bore needle biopsies, yet these advances only became available in the past 18 years. The old large-bore needles provided considerably more tissue for the pathologist to examine, but suffered from compression artifact on the edges and a small but significant risk of needle tracking of cancer; both problems are circumvented with the smaller-bore biopsy needles used today.

The Difficult Case in Prostate Cancer Diagnosis—When is a “Diagnostic TURP” Indicated?
Saturday, 06 May 2006 The technique of transrectal ultrasound-guided core biopsy of the prostate has been improving gradually. It is a well-known fact since the late 1980s, when the “sextant biopsy” with parasagittal sampling became “gold standard,” that aggressive prostate cancer may well be present despite negative findings on digital rectal examination and ultrasonography.

PSA Velocity and Prostate Cancer Detection: The Absence of Evidence is not the Evidence of Absence
Wednesday, 01 March 2006 The article by Schröder et al. [1] touches on an important topic and shows evidence that adds to the current confusion about the role of prostatespecific antigen velocity (PSAV) in prostate cancer early detection and screening: in a prescreened population for prostate cancer (second round) once the serum (total) PSA has reached the cut-off value of 4 ng/ml, the PSAV adds no new information about the presence of a cancer. This is disappointing, considering the urgent need to improve the discriminative power of serum total PSA to separate subjects with prostate cancer from those without.

Saturday, 06 May 2006 Objective: Progression of hormone-refractory prostate cancer (HRPC) is associated with skeletal complications and bone pain, which contribute to deterioration in quality of life (QOL). The effects of new HRPC therapies on patients’ QOL need to be studied. Patient-based assessments that help quantify the risk-benefit profile of HRPC therapies are warranted. This review summarizes the known QOL literature and estimates the potential effect of atrasentan, a novel, selective endothelin A receptor antagonist (SERA), on the QOL of HRPC patients.

Natural History and Treatment of Bone Complications in Prostate Cancer
Wednesday, 01 March 2006 Bone metastases are highly prevalent in patients with prostate cancer, and they commonly present a therapeutic challenge. The natural history of prostatic bone metastases is characterized by skeletal morbidity, often producing distressing symptoms for individual patients and reducing patient autonomy and mobility. These bone metastases are usually radiologically osteoblastic, but there is also a strong osteolytic component as evidenced by marked increases in bone resorption markers. Malignant bone lesions can reduce the structural integrity of the skeleton, resulting in skeletal complications such as pathologic fracture, spinal cord compression, and severe bone pain, which adversely affect quality of life.
