Botulinum Toxin—New Mechanisms, New Therapeutic Directions?

In 2002 the International Continence Society (ICS) published an updated standardisation report on the terminology that describes lower urinary tract function [1]. In this report the overactive bladder syndrome (OABS) or urgency frequency syndrome was defined as ‘‘urgency, with or without urge incontinence, usually with frequency and nocturia’’; urgency was defined as ‘‘the complaint of a sudden compelling desire to pass urine which is difficult to defer’’. Urgency is the pivotal symptom in OABS and drives the other symptoms inherent in OABS, including frequency, nocturia, and urgency incontinence [2]; but is difficult to measure and may arise as the consequence of multiple pathophysiologies. Painful bladder syndrome/interstitial cystitis (PBS/IC), however, encompasses the same spectrum of symptoms but with the additional and pre-eminent triggering symptom of pain.

Tachykinins: Role in Detrusor Overactivity?

The nerves of the lower urinary tract synthetize, store, and release many neuropeptides, including tachykinins, i.e., substance P (SP), neurokinin A (NKA), and neurokinin B (NKB) [1]. It is generally accepted that tachykinins have an important role in afferent signaling from the bladder, and that they may be involved in different pathologies in the lower urinary tract.

Proposed Mechanism for the Efficacy of Injected Botulinum Toxin in the Treatment of Human Detrusor Overactivity

Background: Treatment of human bladder overactivity with intradetrusor Botulinum-A neurotoxin (BoNT/A) injections temporarily blocks the presynaptic release of acetylcholine from the parasympathetic innervation and produces a paralysis of the detrusor smooth muscle. The efficacy of the treatment exceeds that expected from simple detrusor muscle paralysis, however, and its effect of reducing urgency is greatly welcomed by patients.
