EAU 2019: High Power Laser Machines – A Game Changer in the Field of Endourology?

Barcelona, Spain (UroToday.com) Dr. Duvdevani MD, from Hadassah Hebrew University Medical Center, Department of Urology, Jerusalem, Israel, presented data of comparison of two Holmium Laser devices - high power 120W laser machine (Lumenis® MOSES Pulse™120H Holmium: YAG laser, Lumenis Ltd) and in standard 20W (Dornier Medilas® H20 Holmium: YAG laser, Dornier Ltd.).

As an introduction to his talk, Dr. Duvdevani explained the importance of high power laser machines in the modern era of endourology. For this comparison, 462 patients were treated with 20W standard laser and 169 patients with a 120W laser machine. Stone free rate (SFR) was better in the Lumenis group (89.06% vs 81.26% p-value=0.035) and overall laser time was less than half with Lumenis laser vs d20w (195 sec vs 397.14 sec, p-value<0.001) as well. This pattern was demonstrated in all stone densities even for hard stones, above 1200 Hounsfield Units.

Dr. Duvdevani concluded that using new Laser device (Lumenis® MOSES Pulse™120H Holmium: YAG laser, Lumenis Ltd) allows decreasing lithotripsy time approximately half compared to standard 20W laser. This pattern is robust and even exponential when evaluating laser time per stone density, especially in hard stones. Lastly, this retrospective clinical study opens a new horizon of powerful, innovative laser technology that will improve endourology practice and patients care.
