One of the areas of great controversy in the management of couples undergoing Testicular sperm extraction and In vitro fertilization focuses on the perceived benefit of utilizing fresh testicular sperm as compared to cryopreserved sperm . The dilemma for urologists is that the utilization of fresh sperm requires greater coordination of the sperm acquisition with the ovulatory cycle of the partner. This places significant demands on the practitioner and the practice. The current study presented is a collaborative retrospective study which analyzes the impact of Fresh versus Cryopreserved IVF/ICSI cycles in same couples. The authors report no difference in the fertilization rates for fresh versus frozen cycles (55%) in each group. There were no differences in the pregnancy rates or the live birth rates for the two groups .This in conjunction with prior data supports the utilization of testicular sperm which is cryopreserved.

Editorial Comment

Unfortunately this study is not randomized as to the order of the procedures. The fact that couples had a second procedure indicates failure of the first cycle. Since this is a multinstitutional study we do not know if there were differences which were related to the program as opposed to the sperm and procedure.
