ANAHEIM, CA (UroToday.com) - Dr. Barocas reported on the complications of 439 donor nephrectomies (83 right hand-assisted transperitoneal and 356 left pure laparoscopic transperitoneal), which were performed by a single surgeon in their institution. Twenty donors (4.6%) had 21 intraoperative complications. There were 4 conversions (0.9%) and 17 minor complications (3.9%) that were managed intraoperatively without sequelae. Twenty-eight donors (6.4%) had postoperative complications: 6 (1.4%) major complications requiring re-operation or percutaneous procedure and 22 (5.0%) minor complications. There were 12 graft-related (2.7%) complications including 5 graft losses, 3 of which were attributed to recipient factors at explantation. Regression model showed that older patients (p=0.016) and patients earlier in the series (p=0.001) had a higher likelihood of complications. There has been no graft loss in the last 225 cases. The authors should be commended for their excellent reporting, which could add to the currently existing database on the complication rates in laparoscopic donor nephrectomies. This work highlights the fact that although this procedure is now considered the standard of care and is routinely performed in several institutions, attention to details is necessary in order to avoid the complications in these otherwise healthy and altruistic group of patients.
