By injecting several serial boluses of indocyanine green (2.5 mg/ml) each over a several minute period, and then scanning for near infrared fluorescence with a LUNA NIRF system (Novadaq Technologies, Inc. Toronto),the authors were able to accurately depict the margin of a renal tumor in 15 OPEN radical (2) and partial (13) nephrectomies. The system worked well regardless of renal cancer cell type or whether the tumor was cystic or solid. Apparently the indocyanine is readily taken up by the proximal tubules thereby readily differentiating the fluorescent normal renal parenchyma from the nonfluorescent tumor. The authors also noted that tumor laden lymph nodes were likewise nonfluorescent and readily visualized. There were no adverse side effects from administering the indocyanine green.

The authors note that current efforts are aimed at adopting the technology for laparoscopic procedures. To be sure, this would be a great boon for laparoscopic renal surgery as the line of demarcation between normal and tumor tissue could be readily discerned; in addition, a targeted node dissection could be performed in any patient with nonfluorescent lymphadenopathy.
