ANAHEIM, CA (UroToday.com) - Urothelial carcinoma involving the prostate is a relatively frequent occurrence. The authors from Miami investigated the impact of prostatic invasion by TCC by direct invasion (contiguous), or by non-contiguous routes. They studies, 351 men; 71 of whom had prostatic involvement on radical cystectomy. In those with contiguous prostatic invasion the 5 year survival was 12 % compared to 54% in those with non-contiguous prostatic involvement and 66% in those with no prostatic involvement (p<0.001). On Cox regression analysis, pT stage, nodal status and contiguous prostate involvement were independent prognostic factors for disease specific and recurrence free survival. The authors conclude that prostatic urethral TCC without stromal involvement does not affect survival. However, both contiguous and non contiguous prostatic stromal involvement is associated with poor outcome and should be indications for considering adjuvant therapy.
