BERKELEY, CA (UroToday.com) - In the online version of the Journal of Clinical Oncology, Dr. Horwitz and colleagues report ten-year follow-up of radiotherapy (XRT) and androgen deprivation therapy (ADT) in the RTOG-9002 phase III trial.

Patients with T2c-T4N0-X CaP were given 65-70Gy XRT with ADT beginning 2 months prior to XRT and continuing until the end of the radiotherapy. They were then randomized - either to no further ADT or to 24 additional months. ADT included an anti-androgen. Study endpoints included disease free survival (DFS), overall survival (OS), local progression (LP), distant metastasis (DM), biochemical failure (BF), and disease-specific survival (DSS).

There were no differences between groups with respect to stratification factors. In the short-term ADT arm (STADT), grade 3 and 4 toxicities were reported in 9% and 1%, respectively. This was 7% and 1% in the long-term ADT arm (LTADT), respectively. The DFS at 10 years was 13.2% in the STADT arm and 22.5% in the LTADT arm, respectively. The 10-year OS rates were 51.6% and 53.9% for STADT and LTADT, respectively (not significant). LTADT was statistically better for 10-year DFS, DSS and reduced LP, DM, and BF compared with STADT. The primary endpoint testing for an absolute 10% improvement in DFS from 40% to 50% was positive at 5 and 10 years.
