BERKELEY, CA (UroToday.com) - AT1 receptor blockers (ARBs) possessing our experimental demonstrations may offer clinical efficacy in patients with hormone refractory prostate cancer (HRPC). In practice, we conducted a pilot clinical study to examine whether an ARB (candesartan) was able to clinically elicit an antiproliferative effect on hormone-refractory prostate cancer. Surprisingly, a quarter of patients with a PSA decline of more than 50% showed an improvement in performance status. We experienced some cases in which the PSA response was delayed for several months after starting ARB treatment. Thus, we presume that the administered dose of candesartan was too low to overcome multiple metastases. Finally, we believe that ARBs have the potential to delay the PSA increase in cases whose cancer tissues possess AT1 receptors.
