Question: Military veterans and firefighters have been identified at increased risk for bladder cancer. Please compare and contrast the risk factors effecting these patient populations.
Dr. Barry Stein:
“The two groups are really very different. Military veterans have an increased incidence of bladder cancer primarily because they have a high incidence of smoking. This is especially true of Viet Nam veterans since I work at a VA hospital, I see a very high incidence of bladder cancer.
“ Firefighters may not need to be cigarette smokers to be at increased incidence of bladder cancer. They may be seeing toxic chemicals in the fumes in the fires they are fighting.”

We tried NMP22 five years ago when it first came out and after receiving false negatives we elected to not continue its use. Do you have confidence in the sensitivity of the test?
Answer: Dr. Barry Stein
Yes I really do. I think we have a lot of experience now and publications telling us exactly what the false positive and negatives are; for example, someone with gross hematuria or infections or stones where we are likely to see the “false positives.”
I think we have a good idea that this is very sensitive and specific - far more so than cytology. If anybody trusts cytology you have all the more reason to believe this is much more trustworthy as a test and much less expensive.

Do you have an opinion as to when the NMP22 test should be run?
I use the NMP2 test where I would normally use a urine cytology. That leaves two general categories. Category one would be new patients with microscopic hematuria where I have done an upper tract image, usually CT scan, cystoscopy, and some type of examination of the urinary sediment --now it would be NMP22. The other category would be patients who come back for monitoring who have known bladder cancer, and with each of their cystoscopy visits I obtain a NMP22 along with their cystoscopy as part that day’s evaluation.
