p53 Protein Transduction Therapy: Successful Targeting and Inhibition of the Growth of the Bladder Cancer Cells

Introduction: Virus-mediated gene therapy for bladder cancer has some problems, such as efficiency of gene delivery and safety issues. We have reported that poly-arginine peptide (11R) has the ability to increase protein transduction in cells. Here, we show that p53 protein transduction using 11R is useful for targeting to bladder tumors and suppressing the growth of bladder cancer cells.

Watchful Waiting for Low-Grade Ta Bladder Tumours: The Idea would have been Unacceptable 30 Years Ago, but Nowadays We are Highly Interested in the Results of This Study Exploring It

In the 1970s, when intravesical chemotherapy and BCG were developed, doctors advocated more aggressive treatment against superficial bladder tumours. Coagulation alone was unacceptable; random biopsies were obligatory, and an intravenous urography had to be made in all cases. At that time cystectomy specimens were demonstrated to show superficial bladder tumours in about 30% of the cases. Invasive bladder tumours were considered the end stage of superficial bladder tumour disease, and early detection of high-grade tumours could save lives. Later, second resection, early after initial TUR was recommended.

Management of BCG “Failures”

Bacillus Calmette-Guérin (BCG) into the bladder has been demonstrated as an efficacious adjuvant treatment for superficial bladder cancer. BCG is the most effective regimen for the management of superficial disease, including cellular immune stimulation (CIS). However, the treatment fails in 30%–40% of patients, and 30%–40% of those who initially respond relapse. The reasons for an abnormal response may be insufficient or inappropriate CIS or detrimental response because the immune system has been overstimulated [1].

Objective: Defects in the DNA mismatch repair result in microsatellite instability (MSI), which characterise most tumours related to the hereditary non-polyposis colorectal cancer syndrome and some sporadic tumours. Several studies have reported the occurrence of MSI in urothelial cell carcinoma (UCC) of the bladder with a particularly high incidence in tumours from young patients. In this study, we have evaluated the occurrence of MSI in primary bladder UCC arising in seventeen young patients selected for being below 45 years of age at diagnosis.
