Introduction: Isolated retrovesical hydatid cyst is extremely rare. We report on 4 cases of retrovesical hydatid cyst treated at our center in the past 14 years.

Methods: All of the 4 patients were male, aged 55, 67, 63 and 28 years. Two of them complained of compressive syndrome (frequency, nucturia and urgency), other one has a chief complaint of bladder outlet obstruction that led to urinary retention. The last one was asymptomatic and diagnosed accidentally in a patient with a past history of leiomyoma of intestine 18 years ago. A serological test for hydatid cyst has been done in three cases, the result for all of them was positive. Computerized tomography also showed cystic mass in rectovesical pouch in all cases. Cystoscopic evaluation revealed lateral deviation of the bladder neck to the right in one of the cases. In two cases, surgical exploration of pelvic space was done via an intraperitoneal approach in rectovesical pouch; in the two remaining cases, it was done via extraperitoneal incision.

Results: Three symptomatic patients became asymptomatic after operation and pathologic test revealed the hydatid cyst lesion.

Conclusion: Isolated retrovesical hydatid cyst is extremely rare, but in endemic areas we must propose hydatid cyst lesions in differential diagnosis of pelvic cystic masses.
