INTRODUCTION & OBJECTIVES: Recent data (Thompson et al 2004) suggest that routine sextant biopsies will detect large numbers of prostate cancers in PSA ragnes < 3.0 ng/mL. Data from ERSPC are used to evaluate characteristics of PC found in such men by screening after 4 and 8 years and during the intervals.

MATERIAL & METHODS: Within ERSPC Rotterdam 15852 men (79.4% of all) presented with PSA values < 3.0 ng/mL at the 1st screen. Interval PC and PC detected in 11001 and 815 men so far eligible for screening 4 and 8 years later were identified and clinically characterised.

RESULTS: 14 interval cancers surfaced during year 0-4, 2 were staged > T2 and were considered potentially incurable. During the 2nd screen 4 years later 1090 men showed PSA progression to >= 3.0 ng/mL and were biopsied. 275 PC were found (25.2%). 7 (2.5%) were potentially incurable (T>2, Gleason >=7). These included 2 metastatic cases. 
Data after 8 years are so far confirmatory but immature. Updated information will be given in the presentation.

CONCLUSIONS: It seems acceptable to delay the diagnosis of PC in PSA ranges < 3.0 ng/mL. More than 95% of cases detected as interval cancers or after PSA progression 4 years later are still potentially curable.
